1. Approve the Procedure for registration, production and issue of the documents confirming appointment to person of pension which is applied.

2. Determine that the pension certificates issued before entry into force of this resolution are valid before the termination of their effective period or before their replacement in accordance with the established procedure for new.

3. Declare invalid the resolution of board of the Pension fund of Ukraine of March 25, 2004 No. 4-1 "About the Accounting treatment, storages, registrations and issues of pension certificates in the Pension fund of Ukraine and its bodies", registered in the Ministry of Justice of Ukraine on April 06, 2004 at No. 427/9026 (with changes).

4. To legal department (Ryabtseva T. B.) submit this resolution on state registration to the Ministry of Justice of Ukraine.

5. This resolution becomes effective from the date of its official publication.

6. To impose control over the implementation of this resolution on the vice chairman of the board Mashkin V. G.

Procedure for registration, production and issue of the documents confirming appointment to person of pension

I. General provisions

1. This Procedure determines the mechanism of registration, production and issue of the documents confirming appointment to person of pension.

2. Pension certificates are made based on the statement on production of the pension certificate (appendix 1) person who is granted pension which moves in territorial authority of the Pension fund of Ukraine in the place of stay of person on accounting as the receiver of pension.

If person who is granted pension did not reach age of majority or is incapacitated, the application is submitted for production of the pension certificate by parents, adoptive parents, parents tutors, adoptive parents, foster tutors, guardians, custodians in the place of their accommodation / stay, representatives of organizations (guardianship and custody bodies) which perform functions of guardians and custodians (further - legal representatives) on the location of such organizations (bodies).

Pension certificates are made in paper form according to the sample and the description of the form of the paper pension certificate given in appendix 2 to this Procedure or in the form of payment card which at the same time is the pension certificate, contains graphical and electronic information on the owner, qualified certificates of open keys (further - the electronic pension certificate). At the request of person who is granted pension to it the pension certificate electronically (the e-pension certificate) without production of the paper pension certificate or the electronic pension certificate can be created.
