About approval of the Form of the reporting under the device of children in preschool educational institutions and Instructions for its filling

According to article 18 of the Law of Ukraine "About information", I order to subitems 9 and 15 of item 4 of the Regulations on the Ministry of Education and Science of Ukraine approved by the Presidential decree of Ukraine of April 25, 2013 No. 240,:

1. Approve enclosed:

1) the Form of the reporting No. 1 queue "The report on the number of the children staying on the registry for the device in preschool educational institutions";

2) the Instruction for filling of form of the reporting No. 1 queue "The report on the number of the children staying on the registry for the device in preschool educational institutions".

2. Extend the Form and the Instruction specified in Item 1 of this order in the Ministry of Education and Science, youth and sport of the Autonomous Republic of Crimea, departments (managements) of science and education of the regional, Kiev and Sevastopol city public administrations, departments (managements) of formation of district public administrations and executive bodies of city councils.

3. Determine that providing form with the forms and copies of the Instruction specified in Item 1 of this order is assigned to the Ministry of Education and Science, youth and sport of the Autonomous Republic of Crimea, departments (managements) of science and education of the regional, Kiev and Sevastopol city public administrations.

4. To the Ministry of Education and Science, youth and sport of the Autonomous Republic of Crimea, to departments (managements) of science and education of the regional, Kiev and Sevastopol city public administrations annually to provide at the scheduled time collection, control, processing of the reporting and submission of summary information to the Ministry of Education and Science of Ukraine, head departments of statistics in the Autonomous Republic of Crimea, areas, Kiev and to management of statistics in the city of Sevastopol.

5. To department of economy and financing (Danilenko S. V.) annually to provide representation at the scheduled time to Public service of statistics of Ukraine of the summary report in general across Ukraine and by regions.

6. To provide to department of economy and financing (Danilenko S. V.) submission of this order on state registration in the Ministry of Justice of Ukraine.

7. To impose control of execution of the order on the deputy minister of science and education - the chief of staff of Dneprov E. S.

8. This order becomes effective from the date of its official publication.

The instruction for filling of form of the reporting No. 1 queue "The report on the number of the children staying on the registry for the device in preschool educational institutions"

I. General provisions

1. The form of the reporting No. 1 queue "The report on the number of the children staying on the registry for the device in preschool educational institutions" (further - the report) extends to the Ministry of Education and Science, youth and sport of the Autonomous Republic of Crimea, departments (managements) of education and the regional, Kiev and Sevastopol city public administrations, departments (managements) of formation of district public administrations and executive bodies of city councils.

2. In text part of the report all details are filled. The report is signed by the head of relevant organ of management of education or its deputy.

3. If after sending the report to it changes were made, then it is necessary to notify on it addressees urgently.

II. Procedure and terms of submission of the report

1. The report is annually constituted as of December 31 by departments (managements) of formation of district public administrations and executive bodies of city councils and moves to the Ministry of Education and Science of the Autonomous Republic of Crimea, departments (managements) of science and education of the regional, Kiev and Sevastopol city public administrations no later than January 10.

2. The Ministry of Education and Science, youth and sport of the Autonomous Republic of Crimea, departments (managements) of science and education of the regional, Kiev and Sevastopol city public administrations submit the summary report on the region to the Ministry of Education and Science of Ukraine and Head departments of statistics in the Autonomous Republic of Crimea, areas, Kiev and to management of statistics in Sevastopol no later than January 20.

3. The Ministry of Education and Science of Ukraine submits the summary report in general across Ukraine and by regions to Public service of statistics of Ukraine no later than January 30.

III. Procedure for creation of the report

1. In address part of the report are specified full name of body which submits the report, and its location.

2. Data on the children staying on the registry for the device in preschool educational institutions next year are filled in based on the data received from preschool educational institutions according to accounting of applications of parents for acceptance of children in preschool educational institutions.
