About approval of the list, forms, terms of submission of financial and other reporting by fund of social medical insurance for ensuring control functions

According to the subitem 7) article 11 of the Law of the Republic of Kazakhstan of November 16, 2015 "About compulsory social medical insurance", and also the subitem 2) Item 3 of article 16 of the Law of the Republic of Kazakhstan of March 19, 2010 "About the state statistics" PRIKAZYVAYU:

1. Approve:

1) the list, terms of submission of financial and other reporting by fund of social medical insurance for ensuring control functions according to appendix 1 to this order;

2) the "Report on Placement of Assets of Fund of Social Medical Insurance" form according to appendix 2 to this order;

3) the "Report on Assets and Liabilities of Fund of Social Medical Insurance" form according to appendix 3 to this order;

4) the "Arrival Report and Asset Retirement of Fund of Social Medical Insurance" form according to appendix 4 to this order;

5) the "Report on Use of Reserve of Fund of Social Medical Insurance on Covering of Contingencies" form according to appendix 5 to this order;

6) the "Report on Use of Transfers to Fund of Social Medical Insurance on Fee within the Guaranteed Amount of Free Medical Care" form according to appendix 6 to this order;

7) the "Report on Use of Transfers to Fund of Social Medical Insurance on Fee of Subjects of Health Care on Delivery of Health Care in System of Compulsory Social Medical Insurance by the Serviceman, to the Staff of Special State and Law Enforcement Agencies" form according to appendix 7 to this order;

8) the "Report on Fees of the State on Compulsory Social Medical Insurance for Persons Exempted from Payment of Fees in Fund of Social Medical Insurance" form according to appendix 8 to this order;

9) the "Report on Fee of Subjects of Health Care on Delivery of Health Care in System of Compulsory Social Medical Insurance" form according to appendix 9 to this order.

2. Determine that representation of social medical insurance of the regular financial reporting by fund is performed according to the procedure and the terms established by the legislation of the Republic of Kazakhstan on financial accounting and the financial reporting.

3. To provide to department of public procurements and assets of the Ministry of Health of the Republic of Kazakhstan placement of the reporting provided by this order on Internet resource of the Ministry of Health of the Republic of Kazakhstan within ten calendar days after day of submission of the reporting by fund of social medical insurance.

4. To provide to committee of payment of medical services of the Ministry of Health of the Republic of Kazakhstan:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) within ten calendar days from the date of state registration of this order the direction it the copy in paper and electronic type in the Kazakh and Russian languages in the Republican state company on the right of economic maintaining "The republican center of legal information" for official publication and inclusion in Reference control bank of regulatory legal acts of the Republic of Kazakhstan;

3) placement of this order on Internet resource of the Ministry of Health of the Republic of Kazakhstan;

4) within ten working days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan submission to Department of legal service of the Ministry of Health of the Republic of Kazakhstan of data on execution of the actions provided by subitems 1), 2), 3) of this Item.

5. To impose control of execution of this order on the vice-Minister of Health of the Republic of Kazakhstan Aktayeva L. M.

6. This order becomes effective after ten calendar days after day of its first official publication.
