1. Approve the Procedure for registration without registration of personal records in place of registration of the actual accommodation for providing persons with cars with disability and children with disability who are registered as internally displaced persons which is applied.

2. To department of social protection of persons with disability (Polyakova A. Yu.) provide submission of this order on state registration in the Ministry of Justice of Ukraine in the procedure established by the legislation.

3. This order becomes effective from the date of its official publication.

4. To impose control of execution of this order on the deputy minister Fedorovich N. V.

Procedure for registration without registration of personal records in place of registration of the actual accommodation for providing persons with cars with disability and children with disability who are registered as internally displaced persons

1. This Procedure determines the mechanism of registration without registration of personal records by place of registration of the actual accommodation (further - the actual place of residence) for providing persons with cars with disability and children with disability who are registered as internally displaced persons.

2. Person with disability, the legal representative of incapacitated person with disability, the child with disability address to the relevant structural division concerning social protection of the population of district, district public administration, executive body in the cities of Kiev and Sevastopol city, district in the city (except the cities of Kiev and Sevastopol) council (in case of its creation) (further - social security authorities of the population) at the actual place of residence with:

the statement for registration without registration of the personal record for providing with the car in the form given in appendix 1 to this Procedure;

the certificate of the registration of the internally displaced person issued by structural division concerning social protection of the population of district, district public administration, executive body in the city of Kiev city, district in the city (in case of education) council, the employee of the housing and operational organization or the authorized person determined by executive body of village or settlement council in coordination with authorized body and according to Item 2 of the Procedure for registration and issue of the certificate of registration of the internally displaced person approved by the resolution of the Cabinet of Ministers of Ukraine of October 1, 2014 No. 509 "About accounting of internally displaced persons";

the documents determined by Item 34 of the Procedure for the providing disabled people with cars approved by the resolution of the Cabinet of Ministers of Ukraine of July 19, 2006 No. 999.

The social security authority of the population at the actual place of residence of person with disability, the child with disability after registration of a statement in day of the address issues (in case of receipt of the statement by mail - goes to three-day time) the direction in treatment and prevention facility for passing of medical examination and the direction on medico-social commission of experts. After passing by person with disability, the child with disability of medical examination the treatment and prevention facility sends to five-day time in regional, the central city medico-social commission of experts in Kiev the direction (form of primary accounting documentation No. 088/about "the Direction on MSEK", approved by the order of the Ministry of Health of Ukraine of February 14, 2012 No. 110 "About the approval of forms of primary accounting documentation and instructions for their filling used in healthcare institutions irrespective of pattern of ownership and subordination" registered in the Ministry of Justice of Ukraine on April 28, 2012 for No. 661/20974) and the statement from the medical record of the ambulatory (stationary) patient (Form of primary accounting documentation No. 027/about "The statement from the medical record of the ambulatory (stationary) patient", approved by the order of the Ministry of Health of Ukraine of February 14, 2012 No. 110 "About approval of forms of primary accounting documentation and instructions for their filling which are used in healthcare institutions irrespective of pattern of ownership and subordination" registered in the Ministry of Justice of Ukraine on April 28, 2012 for No. 661/20974) and without fail notifies on it the social security authority of the population which issued such direction.

3. After receipt of the notification of treatment and prevention facility on availability at person with disability, the child with disability of medical indications for providing with the car the social security authority of the population directs the documents determined by Item 2 presents of the Procedure, to the relevant structural division of social protection of the population of the Regional State Administration or the Kiev city public administration (further - structural divisions of social protection of the population) at the place of residence of person with disability, the legal representative of incapacitated person with disability, the child with disability.
