1. Approve enclosed:

the application form about inclusion in the register of Authorized Economic Operators;

Procedure for filling of the application form about inclusion in the register of Authorized Economic Operators;

the list of the documents confirming the data specified in the statement for inclusion in the register of Authorized Economic Operators.

2. Determine that the application for inclusion in the register of Authorized Economic Operators is submitted in the form of the electronic document or the paper document.

3. This Decision becomes effective after 10 calendar days from the date of its official publication, but not earlier than the date of entry into force of the Agreement on the Customs code of the Eurasian Economic Union of April 11, 2017.

Procedure for filling of the application form about inclusion in the register of Authorized Economic Operators

1. In the statement for inclusion in the register of Authorized Economic Operators (further respectively - the statement, the register) are specified:

names of the legal entity complete and short (in the presence) applying for inclusion in the register (further - the applicant), according to constituent documents;

the name of the applicant in English;

the location, the e-mail address, contact phone numbers (at the request of the applicant);

number assigned by tax authority (body of state revenues) of state member of the Eurasian Economic Union (further - state member).

2. The applicant one of options of required type of the certificate (types of certificates) by specifying in the respective field of the sign "v" is chosen, at the same time in other fields the crossed out section sign is put down "-".

3. In the Section I of the statement in column 3 of the table are specified:

in Items 1 (including in subitems 1.1 - 1.5), 2, 8 and subitems 11.1 - 11.3 - the values confirming observance of conditions of inclusion in the register;

in Items 3 - 7, 9, 10, subitem 11.4 and Item 12 - marks about observance of conditions in the form of the sign "v" in case of confirmation of observance of the corresponding conditions of inclusion in the register.
