About approval of the Procedure for provision of information from the register of insurers of the State register of obligatory national social insurance

According to part five of Article 17 of the Section V of the Law of Ukraine "About collection and accounting of single fee on obligatory national social insurance", to the subitem 5 of item 4 of the Regulations on the Ministry of Finance of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of August 20, 2014 No. 375 (with changes), I order:

1. Approve the Procedure for provision of information from the register of insurers of the State register of obligatory national social insurance which is attached.

2. In accordance with the established procedure to provide to department of tax policy of the Ministry of Finance of Ukraine:

provision of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.

3. This order becomes effective from the date of its official publication.

4. I reserve control of execution of this order and I assign to the acting as the Chairman of the Public fiscal service of Ukraine M. V Is sold.

Procedure for provision of information from the register of insurers of the State register of obligatory national social insurance

I. General provisions

1. This Procedure is developed according to part five of Article 17 of the Section V of the Law of Ukraine "About collection and accounting of single fee on obligatory national social insurance" (further - the Law).

2. Are determined by this Procedure:

the procedure of forming and provision of the statement from the register of insurers of the State register of obligatory national social insurance (further - the register of insurers) at the request of payers of single fee on obligatory national social insurance (further - payers of single fee) and/or insured persons;

request form about receipt of the statement from the register of insurers;

form of statements from the register of insurers;

form of the certificate of lack of data in the register of insurers.

3. In this Procedure terms are used in the values given in the Law and the Law of Ukraine "About state registration of legal entities, physical persons entrepreneurs and public forming".

4. According to Item 5 of the Section II of the Provisions on the register of insurers approved by the order of the Ministry of Finance of Ukraine of November 24, 2014 No. 1162, registered in the Ministry of Justice of Ukraine on December 3, 2014 at No. 1554/26331 (with changes), the Public fiscal service of Ukraine daily publishes on the official web portal data from the register of insurers on statement on accounting/removal from accounting of the payer of single fee and its class of professional risk of production (for the period till January 1, 2016).

If more complete information from the register of insurers concerning the payer of single fee is necessary for the payer of single fee and/or insured person, than is promulgated on the GFS official web portal, or documentary confirmation of the insurers this the register is necessary, then in that case the payer of single fee and/or insured person can receive the statement from the register of insurers according to the procedure, determined by the Section II of this procedure.

II. Forming and provision of the statement

1. The request about receipt of the statement from the register of insurers is sent in form No. 1-ZhRS (appendix 1) one of the following methods:

personally the payer of single fee and/or insured person or person authorized on it in territorial authority of the Public fiscal service of Ukraine (further - monitoring body) in the main place of accounting of the payer of single fee or in other monitoring body;

goes by mail to monitoring body in the main place of accounting of the payer of single fee or to other monitoring body;

electronically means of electronic communication.

The payers of single fee who signed the agreement on recognition of electronic documents with the relevant monitoring body can send request means of electronic communication electronically with observance of requirements of the law on electronic document management and the digital signature.

Insured persons can send request means of electronic communication electronically with observance of requirements of the law on electronic document management and the digital signature.

All Sections of request are subject to filling. The request shall be signed by the applicant with indication of date.

2. At the request of the payer of single fee and/or insured person the monitoring body provides free of charge within two working days following behind day of receipt of such request, the statement from the register of insurers or the certificate of lack of data in the register of insurers.

3. The statement is created of the register of insurers by the corresponding criteria of search and contains data from the register of insurers who are urgent for date of forming of the statement.

Depending on category of payers one of such statements is created:

the statement from the register of insurers (concerning the legal entity or separate division) in form No. 1-BPC (appendix 2);

the statement from the register of insurers (concerning physical person entrepreneur or physical person which performs independent professional activity) in form No. 2-BPC (appendix 3);

the statement from the register of insurers (concerning the physical person taking voluntary part in system of obligatory national social insurance) in form No. 3-BPC (appendix 4).

The statement is effective before modification of the register of insurers regarding the data concerning the insurer on whom the request is provided.

4. In the statement from the register of insurers (concerning the legal entity or separate division) in form No. 1-BPC are specified:

tax number of the payer of single fee;

name of the payer of single fee;

the name of monitoring body where the payer of single fee is/was on accounting;

code and name of category of the insurer;

registration number of the payer of single fee;

date of registration of the payer of single fee;

date of removal from accounting of the payer of single fee;
